Call the Embassy at 202-332-7100 ext. 3038 to request forms by post - ETD forms are not available online at this time.   Please note that each person is only entitled to one (1) ETD, which is valid for six (6) months from the date of issue. Within that six-month period, one must make all the necessary arrangements for a new passport.     1.                  Completed P1(A) and P34 forms   2.                  Copy of Birth Certificate   3.                  Copy of Zimbabwe ID Card   4.                  Copies of expired passport: pages 1-5 of old-type passport or last page of new-type passport (or police report for lost passport)**   5.                  Proof of pending passport application, e.g., copy of receipt   6.                  Copy of confirmed itinerary   7.                  Two passport photos   8.                  US$73.00 money order payable to the Embassy of Zimbabwe (personal checks not accepted)   9.                  Self-addressed, stamped return envelope (Express Mail or Priority Mail)   10.              Send documents to the above address   11.              Processing time is 5 business days   12.              Incomplete applications will be returned unprocessed     **For nationals in custody pending deportation by US Authorities, submit ORIGINAL ZIMBABWE PASSPORT for them to be issued with an ETD.
